Yes, there is the IdentityIQ Application Onboarding Questionnaire which is part of the Services Standard Words (SSW) - A collection of guides and template documents that aid in the management of an IdentityIQ deployment project that can be found among the items here with the Services Standard Deployment.

If you download the Services Standard Deployment (SSD) v7.0.2 you will find the template in the folder doc\Templates\Design.
